BACKGROUND: Here we evaluate auto-antibody response against two potential antigenic determinants of genes highly expressed in low Gleason Score prostate cancer (PC) tumor samples, namely FLJ23438 and VAMP3. METHODS: RT-PCR assays were used to analyze mRNA expression profiles of FLJ23438 and VAMP3 transcripts. The auto-antibody response against FLJ23438 and VAMP3 recombinant proteins was tested by immunoblot assays using PC, benign prostate hyperplasia (BPH), healthy donors (HD), and other human cancers plasma samples. RESULTS: Our data showed that 37% (10/27) and 7.4% (2/27) of PC plasma samples presented auto-antibodies against FLJ23438 and VAMP3, respectively. Only 8.3% (1/12) of BPH plasma samples were reactive for both auto-antibodies, while none (0/12) of HD plasma samples tested were reactive. CONCLUSIONS: The prevalence of 37% of positive PC plasma samples for anti-FLJ23438 antibodies suggests that humoral immune response against this antigenic determinant could be a potential serum marker for this cancer.

PURPOSE: To evaluate the results of hydroxylapatite augmentation of the mandible followed by placement of the mandibular staple. PATIENTS AND METHODS: Sixty patients 5 or more years postsurgery were evaluated. Patients were followed yearly with clinical and radiographic examinations. RESULTS: The decrease in augmentation height was 2.8 mm (33%) after the first year and only minimal in the following years. The following problems were seen: hyperplastic tissue around posts (eight patients), small dehiscences were seen in the saddle area after 2 years (11 patients), large dehiscences with significant hydroxylapatite loss in five patients. One patient needed a secondary augmentation. CONCLUSION: Prosthetic stabilization and function were excellent throughout the study period and there was a uniformly high degree of patient satisfaction.

In a review of 102 consecutive cases of the fixed mandibular implant there were no failures; all appeared clinically to be osseointegrated and functioning well. There were an unexpected number of gingival infections and instances of inflammatory gingival hyperplasia, but these have been reversible and manageable. A change in sleeve nut design has improved gingival health. The Fixed Mandibular Implant (Hall Reconstructive Systems, Carpinteria, CA) offers immediate denture stability and function, and provides the opportunity for a fixed, fixed removable, or overdenture prosthetic reconstruction.

The main advantages of the FMI are immediate denture placement and function, the versatility for denture construction, and the absolutely parallel abutments that make prosthetic adaptation easy. During the submental approach, the oral and maxillofacial surgeon can perform cosmetic surgery of the submental fat pad and chin. There is usually less pain and swelling as compared with the intraoral dental procedure for placing four or five implants. The FMI is an excellent option for the anterior edentulous mandible or deformed mandible when fixed or fixed removable dentures or overdenture is needed. The team approach allows the surgeon and prosthetic dentist to combine their skills in the management of edentulous patients.

The staple bone plate implant has been used to stabilize and retain an mandibular overdenture. This implant-with-pin configuration, with four pins penetrating the bone and mucosa, was used to support a mandibular fixed complete denture. The prosthesis was completely implant supported. All of the occlusal loads were directed to the bone through the implant pins. Results for six patients were reported. The implant and the prosthesis were well tolerated.

The mandibular staple bone plate is a reconstructive device that will attach to the inferior border of the mandible and will orthopedically provide for prosthetic and jaw stabilization in the edentulous mandible and the deformed mandible. It can be useful in a variety of clinical settings and is adaptable to a number of prosthetic needs. It is cost-effective and has demonstrated osteointegration in over 91 per cent of cases. The cumulative rate of success for 5 to 6 years is 94.6 per cent.

A retrospective study was done of 1,516 patients who had a mandibular staple bone plate operation for jaw reconstruction during the period from 1968 to 1984. The results indicated that 91.75% of the staples were functioning well without major complications, 2.70% were functioning well following repair or replacement, 1.98% were functioning moderately well after unloading (overdenture relief due to mobility), and 3.57% had been removed. The cumulative rate of success was 94.64% for five to six years and 90.89% for ten to 16 years.

A prospective study that evaluated the surgical and postsurgical problems of 9,574 patients of a wide range of ages who had had 16,127 third molars removed was performed. It was concluded that removal of mandibular third molar teeth during the teenage years resulted in decreased operative and postoperative morbidity.

Two cases of aggressive intraosseous epithelial tumor of the jaws are presented. Both tumors demonstrated an unusual biphasic pattern with areas of acceptable follicular ameloblastoma together with a conspicuous clear-cell component. The clinical course indicates these lesions should be considered as low grade odontogenic carcinomas. Further subclassification according to the World Health Organization classification and the recently proposed modification by Elzay is discussed.

A case of intramuscular hemangioma of the tongue originally suspected to be an epidermoid or dermoid cyst was presented. The correct diagnosis was discovered during surgery, when uncontrollable bleeding and the results from a frozen section revealed the true nature of the lesion.

A case has been presented of reconstruction for a patient with massive injuries of the face and jaw. Reconstructive surgery, including bone grafting, skin grafting, and placement of the mandibular staple bone plate, and prosthetic rebuilding of eyes and dentures have made him a reasonably functioning and cosmetically acceptable individual.
